OCF Forms
New OCF Form Downloads Now Available!

Financial Services Commission of Ontario (FSCO) updated their Auto Insurance Claim Forms (OCF Forms) as of January 7, 2008.
 
ABELMed Inc. created new OCF MS Word fillable forms that can be used with the Letter Manager feature within your ABELMed program.
 
To download the OCF forms:
  1. Locate the OCF Form that you require from the list below.
  2. Right click on the OCF Form link.
  3. Select Save Target As from the menu. 
  4. Save the file to your Desktop. 
Copy the file from your Desktop to the folder named ABELMed\Data\Letters located on your ABELMed server computer. You may replace the existing OCF Form with the newly downloaded copy.

Changes were made to the Ministry of Health and Long Term Care Fee Schedule effective October 1st, 2009.
